The Cleveland Avenue Dental Center is part of the Forsyth County Department of Public Health. We serve children and adults of Forsyth County and surrounding areas. We accept Medicaid, NC Health Choice, Ameritas and self-pay patients. A sliding fee scale may be applied for self-pay patients based on household income.
